Referral Process
Referrals come from a variety of sources e.g. Social Services, Education, Health, Contracts and Commissioning Officers and often directly from parents. If there is a place available it is usual for parents and social workers to visit the home to ensure that it is the most suitable placement. We welcome this as it also allows for parents to meet the Registered Manager of the home and the key staff members who will be working with their child and to get a feel for our ethos and values.
The next step would be for Living Ambitions to arrange an initial assessment which begins the planning process. Following written confirmation of an agreement of placement, the detailed planning process can begin which may involve all the people within the child/young person's life, both personal and professional. All planning processes are person centered and individualised according to circumstance and need. This equips the staff team with as much information as possible about the child/young person with the completion of documents such as risk assessments, ‘My Personal Book’ and a sensory profile.
