Person Centred Planning
Services are designed and developed to the individual’s own specification based on a planning process that puts the person first using the principles of Person Centred Planning (PCP).
By using the PCP process we can ensure that each person's rights are respected, independence is supported, choices are encouraged and maintained, and inclusion in the family, neighbourhood and broader society are championed by a well trained person centred team of professional support workers and manager's.
You might ask "well, what is Person Centred Planning?", and we’d say it’s the following:
Person Centred Planning is a set of ways of planning your life.
It is called 'Person Centred' because:
- It puts you in charge of planning your life
- Finds out what matters to you
- Thinks about your gifts, and what people like and admire about you
- Listens to you and the people who care about you
- Finds ways to move toward your dreams
- You and the people round you learn and grow together
It can be summed up in the following quote from Michael Smull & Helen Sanderson, two people who have done a lot of work about Person Centred Planning which says:
"Person Centred Planning is a process of learning how a person wants to live and then describing what needs to be done to help the person move toward that life"

Living Ambitions works with people in ways that ensure we have an understanding of exactly what you want from your life and we work in ways, sometimes called person centred team’s, that ensures this will happen. We are experienced at facilitating PCP for people or working to plans that have been facilitated by others. Your PCP is important in lots of ways and not least because it should inform everyone involved in your life how you want to live your life.
Everyone we support has a PCP and this informs us of what sort of service design is needed to fulfil and sustain your life in your supported living service.
