What’s next?....Moving On-Growing Up-Coming Back Home
Transition occurs when people move from one area of their life to another. This can happen with young people moving from services designed for children to services designed for adults.
For adults who have lived at home with their parents for a number of years and for one reason or another want to move to either a more independent lifestyle away from the family home or who want to move into a supported living setting which they may share with other people.
Or for people who have lived outside of their home area in a residential school or home or hospital setting and who want to move back ‘home’.
This is where it gets really exciting, now it’s time to choose what you want to learn, to plan for your future and acquire the skills to move you forward in to the next stage of your life.
More choice, more responsibility, you want to make good choices that are right for your future and to do this effectively you need to understand your options fully and think carefully what is best for you.
These are big decisions and there are lots of people out there; your family, carers, friends teachers who can help and advise you. Living Ambitions are skilled facilitators of Transitional Plans and can either be part of your planning team or the take the lead at facilitating and developing your plan in partnership with people who mean something to you.
