Q. What exactly is Transition?
A.
Transition is the planned movements of you as a young person with physical difficulties, learning disabilities and sensory impairments from child centred services to adult oriented services.

Q. When can my Transition process start and who is involved?
A.
The process begins when you reach age 14, and is usually initiated by your school. A Transitional Plan is then developed. This will be jointly co-ordinated by your school, together with all necessary agencies, including your families/carers. If your school does not initiate the plan Living Ambitions is sometimes asked to do this independently.

Q. What will happen then?
A.
The Children’s Disability Team will assess your needs and the needs of your family under the National Children’s Framework Assessment. The Adult Community Care Team will use a unified assessment to evaluate and identify your needs. Findings from both assessments will help decide if you or your family are eligible to receive our support and identify appropriate services that may help.

Q. How can Living Ambitions help me in this process?
A.
We can get involved at any stage from your review when you are 14 upwards. We can help to facilitate your Transition Person Centred Plan and because of our experience, we can provide a wide variety of person centred services for you in your family home. We are also able to support you when you are older to move into supported living, or assist you and your family to live together in the family home. Because we are person centred we work hard to make sure what we do is what you and your family want, and we do this by continually checking and reviewing your plans with you.

Q. What is a Transition Plan or Person Centred Plan?
A.
A Person Centred Plan is a way of developing your Transition Plan that ensures you are at the centre of the process. By using person centred approaches the services we provide will reflect your needs and goals, and take full account of you and your family’s expressed aspirations. The Person Centred Plan will continued to be reviewed and added to as you progresses into adult life and will ensure that the services being provided are done in a way that continues to enhance the your physical, mental and social well being to allow you to contribute and be a part of your local community in the fullest sense.

Q. When my Transition Plan is completed what happens next?
A.
You may have decided that you wish to live a more independent life so Living Ambitions will work in partnership with you and your family, social worker, health worker and other key individuals who make up your circle of support.

We are able to provide a wide variety of person centred support for people in various circumstances for example, whilst people live within their family home, in setting up their new home, going to college or work placement, even assisting the person to identify a housing provider, as Living Ambitions have very positive relationships with housing associations and providers.
